Now you can.  Follow these simple instructions and YOUR report will be added to this page, along with your detailed report for other SACK members, or people cruising our clubs website.

• Click on the SACK Repair Facility Evaluation form link here:   SACK_RFE
• The form will download and open in Adobe which is a free program.  If you don't have Adobe Reader, You can download it here:  Adobe
• Fill in the form and click File, then Save As.
• Save the file to your desktop.
• Open an email and attach the saved file to your email.
• Send the email to Frank (onemileshigh@msn.com) and he will compile and report your evaluation on the website.

• Alternatively, if the above doesn't work you can download a free program at this link, Cute PDF  and after you install this program you can print to pdf and send the file back to Frank via email.

• If neither of the above two solutions work, you can send Frank an email with the details of your experience and he will fill out the form and report it on the website for you.
